What happens during a consultation with a pediatric pneumologist?

First consultation
During the first consultation, the doctor will ask you about your child’s symptoms and medical history, and perform an examination. If your child is over 5 years old, the doctor will perform a spirometry, a test that determines the overall health of the lungs.

Depending on your child’s situation, sleep apnea screening can be done at home by performing nighttime oximetry, a test used to analyze the oxygen content of the blood during sleep. Your pneumologist will give you detailed instructions should your child require this test.

If needed, your child will be seen for follow-up visits.

Follow-up visits
During follow-up visits, the doctor will perform a spirometry test (for children aged 5 years and older) to assess changes in your child’s lungs since their last visit. They may recommend other tests, if needed. Please feel free to ask questions or express any concerns.

Circumcision

Dr. Slim’s many years of experience performing circumcisions means that he goes the extra mile to ensure that you get the best care and comfort.

Circumcision is a surgical procedure that removes the skin (foreskin) covering the head of the penis (glans). This operation is performed under local anesthesia and lasts only a few minutes. Rest assured, you or your child won’t feel a thing.

Circumcision can be performed on males as young as 2 years of age. The younger the child, the easier the operation (as they are less aware of the procedure). That is why we advise all parents wishing to have their child circumcised to begin the procedure as soon as possible.

The doctor applies an anesthetic cream on the penis with the child lying down comfortably. The cream’s effect lasts 20 to 30 minutes.

Once the skin is numb, your child will receive an injection of dorsal penile ring block, an anesthetic, into the soft tissue surrounding the penis. This combination block of short- and long-acting anesthetics allows your child to go pain-free both during and in the hours following the procedure. The anesthetic cream ensures that your child does not feel anything during the injection.

After about 10 minutes, the penis is completely desensitized, and the circumcision can take place. The circumcision itself lasts only a minute.

During the injection and circumcision, your child is given a sugar lollipop to distract him and reduce his perception of pain.

It takes 7 to 10 days for the penis to heal. After the procedure, you will receive detailed instructions to minimize your child’s discomfort and ensure optimal healing. We are also available by phone 24 hours a day to address any concerns you may have.

ABPM Exam

The measurements are obtained using a portable device consisting of a cuff attached to your arm and connected to a monitor that is worn on your waist or on a shoulder strap.

Your blood pressure constantly changes throughout the day, depending on your activity and other factors. Results taken in a doctor’s office do not always reflect reality, and often tend to be higher due to “white coat syndrome”. ABPM provides a more reliable reading by measuring your average blood pressure under normal circumstances.

Your doctor may recommend ABPM to assess any of the following:

  • Suspicion of early high blood pressure
  • Assessing the effectiveness of high blood pressure treatment
  • To ensure that you do not have low blood pressure.

What happens during an ABPM exam?

On the day of your appointment, the attending nurse will attach a cuff to your nondominant arm (i.e. if you are right-handed, the cuff will be placed on your left arm; if you are left-handed, it will be placed on your right arm). The cuff is connected to a recording device programmed to measure your pressure at 20-to-30-minute intervals during the day and every 60 minutes at night.

The device is inserted into a case and attached to your waist or placed on a shoulder strap. Once in place, the nurse let you know when to come back to the clinic and give you an event diary in which you should note the following:

  • Bedtime, wake-up time, physical activity, nap time
  • Times you eat, drink alcohol, drink tea/coffee/energy drinks
  • Times you take your blood pressure medication, if applicable
  • Any symptoms you may experience.

It is important to carry on with your normal routine while wearing the monitor. Please avoid showering or bathing, as this will damage the device. You may place the device at your side when sleeping.

Please note that when you feel the cuff inflate, be sure to let your arm hang down and stay still. The measurement is over once the cuff has completely deflated; at which time you can resume your normal activity.

Return of the device

Your ABPM will be removed by the attending nurse at the scheduled appointment time.

Results will be reviewed by a cardiologist and promptly forwarded to your referring physician.

What happens during a consultation with a social worker?

First consultation
During the first visit, the social worker will ask you questions to better understand your child’s situation. They will then suggest solutions and develop a treatment plan. We offer both individual and family sessions, depending on the specific issue.

If your child’s situation requires it, they may be referred to another one of our team’s many specialists or to other relevant community organizations.

Follow-up appointments
During follow-up visits, we assess changes since the previous consultation and review the treatment plan with you. We can also suggest other resources that may be helpful.

What happens during a consultation?
During the consultation, the doctor assesses your child’s health and any vaccinations they may have received in the past. You will be asked questions about your vacation destination and the duration of your stay. The doctor will advise the following depending on the country you plan to visit:

  • Outline the required vaccines and possible reactions following vaccination
  • Explain the risks and the necessary precautions to protect your child’s health and safety
  • Give practical, up-to-date advice about water, food, mosquitoes, swimming, sun, etc.

Your child will then receive the required vaccinations. The doctor may also prescribe certain medications, depending on the situation.

Repetitive Transcranial Magnetic Stimulation (rTMS)

There are many ways to treat depression, including psychotherapy, medication or a combination of several therapeutic approaches. Repetitive transcranial magnetic stimulation (rTMS) is among the the most innovative solutions for people suffering from depression. It also happens to be one of the least known treatments.

Approved since 2002 by Health Canada for the treatment of depression, rTMS is scientifically validated according to Canadian practice guidelines and intended for patients suffering from depression and who fail to respond to a first round of antidepressants.

What is rTMS?

A coil is applied to the user’s scalp in order to send magnetic pulses that induce electrical currents in the neurons of the targeted area of the cortex. During the procedure, which lasts about 15 minutes, the user is awake and no anesthetic is required. Patients can return to their normal routine, including driving afterwards, as procedures do not entail any rest or recovery period. Treatments are performed twice a day, 3 to 5 days a week, for a total of 30 procedures.

Effectiveness

rTMS has been shown to be effective in the treatment of depression, with a positive response rate of up to 50%. Most users see an improvement in their symptoms after a few sessions.

How the Consultation Process is Carried Out

This treatment is not intended for all patients suffering from depression, and an evaluation by a psychiatrist is required beforehand.
