Many factors can affect your energy, sleep and well-being. A health assessment can help you understand what’s going on and decide on the next steps in your care.
What’s included
- One health assessment
- Two 10-minute follow-up appointments with a nurse
- Two 20-minute follow-up appointments with a doctor to adjust your hormone therapy, if needed
- Annual membership, administration fees and help coordinating specialist follow-up when needed
- One set of lab tests; additional tests billed separately
You can give one of your follow-up appointments to someone aged 14 or older who lives in Québec. The appointment covers a single health concern and cannot be used for a comprehensive health assessment.
Your doctor will recommend blood tests based on your needs. Each package includes one set of lab tests. Any additional tests are billed separately.
