1. Who this policy applies to

This policy explains how ELNA Private Care, operated by ELNA Medical Group Inc. (“ELNA”, “we”), handles your personal information on this website and when you receive our services. It supplements the ELNA privacy policy, which applies to all ELNA services. Please read that policy as well.

We handle your information in accordance with Québec’s Act respecting the protection of personal information in the private sector, as amended by Law 25, and the rules that govern medical records.

2. What this website collects

This website provides information only. It has no forms, patient accounts or online booking.

Our hosting provider may keep technical logs, including your IP address, browser type, the page requested, and the date and time, to keep the website secure and working properly. Fonts are hosted on our own server. Google Maps opens only when you click a map link.

3. Cookies and advertising

Analytics and advertising cookies are off by default. They are activated only if you consent to the relevant category in the cookie banner:

• Essential: one cookie that records your choices for 6 months. Always on.
• Analytics: Google Analytics helps us improve the website by showing us which pages are viewed, the type of device used and the approximate region. Google’s ad personalization signals are turned off.
• Advertising: Google Ads and the Meta pixel (Facebook, Instagram) help us measure ad performance, such as calls or clicks on booking links, and show ads to people who have visited the website.

Advertising cookies are never used on our pages about a specific health condition (ADHD assessment, hormonal health, STI testing, gynecology, collective prescriptions and laboratory), so that no advertising platform receives information about your interest in a health condition. We do not use your health information for advertising.

Google and Meta may process this data outside Québec under their own privacy policies. You can change or withdraw your consent at any time using the “Manage cookies” link at the bottom of any page. You can still use the website if you decline cookies.

4. When you call or write to us

When you contact us at 514-787-0055 or info@elnamedical.com, we collect the information you give us, such as your name, contact details and the reason for your request, so we can respond and arrange your appointment.

Regular email is not secure. Please don’t send sensitive medical information by email. Our team will explain how to share it securely.

5. When you receive care

To provide your care, we collect information including:

• your identity and contact information;
• your health information: history, reasons for consultation, examinations, results, diagnoses and treatments, recorded in your medical record;
• your insurance information, if you request direct billing;
• the information needed to process payments and issue receipts.

We use this information to provide care, follow up on your results, coordinate specialist appointments when needed, send appointment reminders, process billing and issue receipts. We do not use it for targeted advertising.

6. Who your information is shared with

Your health information is only shared with people who need it for your care, or with your consent, except where the law provides otherwise:

• the physicians, nurses and staff involved in your care;
• CDL Laboratories, for sample collection and testing, and the specialist clinic when you are referred there;
• your insurer, if you request direct billing;
• our service providers, such as IT and electronic medical record providers, who are contractually required to maintain safeguards equivalent to ours.

Your employer never receives your results, diagnoses or reasons for appointments, including when you access care through an employer offer.

If information had to be shared outside Québec, we would first carry out a privacy impact assessment, as the law requires.

7. Partners and third-party websites

When you book a collective-prescription service at testlabo.ca, your information is collected by CDL Laboratories under its own privacy policy. The same applies to Google Maps and any other website we link to.

8. Security and retention

We protect your information with physical, technical and administrative measures, such as controlled access to premises and systems. No transmission method is completely secure, but we take reasonable measures to protect your information. If a confidentiality incident presents a risk of serious injury, we will notify you and the Commission d’accès à l’information.

We retain your medical record for the period required by the Collège des médecins du Québec. Other information is kept for as long as needed for the purpose for which it was collected, then securely destroyed or anonymized.

9. Your rights

You can:

• ask for access to the information we hold about you, including your medical record;
• ask for correction of inaccurate, incomplete or ambiguous information;
• withdraw your consent, subject to legal obligations and what is essential to your care;
• request a copy of your information in a structured, commonly used technological format;
• decline promotional communications.

Contact our privacy officer (section 11). We will verify your identity and reply within 30 days of receiving your request. If you are not satisfied with our reply, you can contact the Commission d’accès à l’information du Québec.

10. Patients under 18

In Québec, patients aged 14 or older can consent on their own to care required by their state of health. Their health information is kept confidential. For children under 14, including those receiving laboratory services, a parent or guardian provides consent.
