Terms and
conditions.
Terms for using the ELNA Private Care website and for our services, memberships and packages.
Last updated: October 5, 2026
Part 1 · Use of the website
1. Acceptance of these terms
The ELNA Private Care website (the “website”) is operated by ELNA Medical Group Inc. (“ELNA”, “we”). By using the website, you accept these terms and conditions and the privacy policy, which forms part of them. If you do not accept them, please do not use the website.
We may update these terms. The current version is published on this page with its last updated date. Changes do not apply to a package or membership you have already purchased unless you agree to them.
2. General information, not medical advice
The information on this website does not replace a consultation, assessment or advice from a health care professional. Speak with a doctor or another qualified professional before making decisions about your health.
In an emergency, call 911 or go to the nearest emergency department. For non-urgent health advice, you can reach Info-Santé at 811, 24 hours a day.
3. Accuracy of information and prices
We make reasonable efforts to keep information about our services, hours and prices accurate and up to date. All prices are in Canadian dollars. The price of your visit or package is the amount our team confirms when you book or make a purchase.
Prices for services offered by partners, such as the collective-prescription services of CDL Laboratories, are set by those partners and may change.
4. Partners and third-party websites
The website links to third-party websites, including testlabo.ca and cdllabs.com (CDL Laboratories) and Google Maps. Those websites are governed by their own terms and privacy policies. Services booked with CDL Laboratories are provided by CDL Laboratories, in partnership with ELNA.
A link to a third-party website does not mean we endorse all of its content. We are not responsible for the content or operation of those websites.
5. Permitted use
You may use this website for lawful, personal purposes. You must not:
- access or attempt to access content, data or systems not intended for you;
- disrupt the website’s operation or security, including through viruses, cyberattacks or deliberate overloading;
- use a robot or other automated means to copy website content without our written permission;
- impersonate ELNA, a member of our staff or another person;
- transmit false or misleading information, or illegal, hateful or harassing content.
6. Intellectual property
Website content (text, graphics, logos, layout and design) belongs to ELNA or its licensors and is protected by intellectual property laws. The ELNA names and logos are trademarks of ELNA or its affiliates. The CDL Laboratories names and logos belong to their owner.
You may view the website and print a page for personal use. Any other reproduction, modification or distribution, in whole or in part, requires our written permission.
7. Personal information and communications
We protect your personal information in accordance with Québec’s Act respecting the protection of personal information in the private sector and the rules that apply to medical records. The privacy policy explains what information we collect, why, and how to exercise your rights of access and correction.
This website has no forms for submitting medical information. Analytics and advertising cookies are activated only with your consent. Advertising cookies are never used on pages about a specific health condition. See our privacy policy for details. Regular email is not secure, so please don’t send sensitive medical information by email. Contacting us does not confirm an appointment.
8. Website availability
We may change, suspend or remove all or part of the website, including for maintenance or security reasons. We do not guarantee that the website will always be available, uninterrupted or error-free.
9. Liability
To the extent permitted by law, ELNA is not liable for indirect damages arising from use of the website, inability to use it, or use of a third-party website.
Nothing in these terms limits ELNA’s liability for bodily or moral injury, for intentional or gross fault, or the rights granted to you by Québec’s Consumer Protection Act or the Civil Code of Québec.
Part 2 · Services, membership and packages
10. Nature of the services
ELNA Private Care offers private primary care for patients aged 14 and older, subject to availability and each service’s requirements. Laboratory services are available to patients aged 3 and older.
ELNA Private Care consultations are paid services and are not covered by RAMQ. The public specialist clinic operates separately. Its services are covered by RAMQ for eligible patients, require a medical referral and are subject to availability. ELNA membership does not guarantee registration with the family medicine group (GMF), priority access or a specific wait time to see a specialist.
Care and treatment are based on your medical assessment. No specific diagnosis, treatment, prescription or outcome is guaranteed.
11. Appointments
Call 514-787-0055 to book an appointment. Your appointment is not confirmed until our team confirms it with you. For nurse-led testing services, book online with CDL Laboratories at testlabo.ca.
For minor urgent health concerns, appointments are generally available within 24 hours, or the same day if you contact us before noon. These timeframes are targets and are not guaranteed. This service does not replace emergency care.
12. Annual membership and individual appointment fees
The $129 annual membership is optional. It covers one person. Members save $60 per visit on individually booked medical services, including health assessments, consultations and minor urgent care. The discount does not apply to CDL laboratory tests. Membership is valid for 12 months from the date of purchase. Annual membership is included in Alliance Santé, Alliance Signature, Équilibre 360, ADHD and ADHD+ packages.
Individual appointment fees: minor urgent care or follow-up (20 min): $175 member, $235 non-member; complex follow-up appointment (30 min): $225 member, $285 non-member; health assessment and care plan (about 45 min): $285 member, $345 non-member.
Non-members pay $60 more per visit. Membership starts saving you money on your third individually booked visit within the year.
Membership is for your own use and cannot be transferred.
13. Alliance Santé, Alliance Signature, Équilibre 360 and ADHD packages
Details and prices for Alliance Santé, Alliance Signature, Équilibre 360, ADHD and ADHD+ packages are available on the service and pricing pages. These prices include annual membership and administration fees.
Each package covers one person and is valid for one year. Your doctor will recommend blood tests based on your needs. Each package includes one set of lab tests. Any additional tests are billed separately.
Alliance Signature. Unlimited minor urgent-care visits are for the member’s own medically necessary care, subject to availability and reasonable use. Follow-up care is limited to the two appointments included in the package.
Family discount. If you have a valid package, your spouse or an immediate family member aged 14 or older can get 10% off their own package.
Transferable follow-up visit. You can give one of your follow-up appointments to someone aged 14 or older who lives in Québec. The appointment covers a single health concern and cannot be used for a comprehensive health assessment.
Included test (Alliance Santé and Alliance Signature). One test per year under a collective prescription at any CDL location, up to a value of $150. If your chosen service costs more, you pay the difference.
14. Nurse-led testing and CDL Laboratories
You can get tested for urinary tract infections, pregnancy, strep throat, sexually transmitted infections (STIs) and HIV without a doctor’s requisition through services provided under collective prescriptions. Book an appointment with a nurse to check your eligibility and have the appropriate tests done or arranged. Treatment is prescribed when needed, in accordance with the applicable protocol.
These services are provided at CDL locations in partnership with CDL Laboratories. Each service begins with an eligibility check. If you don’t meet the criteria for care under a collective prescription, you’ll be directed to a medical consultation. CDL Laboratories’ booking terms apply.
15. Payment, receipts and insurance
You are responsible for the cost of your ELNA Private Care services. We provide receipts for eligible services and offer direct billing with participating insurers. You pay any amount your plan does not cover.
Coverage depends on your insurance plan and is not guaranteed. Check with your insurer before your visit. Some expenses may qualify for the medical expense tax credit; keep your receipts.
16. Cancellation, renewal and refunds
Before you book or make a purchase, we explain accepted payment methods and the terms for changing or cancelling appointments, renewing memberships or packages, and requesting refunds. To change or cancel an appointment, call our team as soon as possible.
These terms apply subject to the rights granted to you by Québec’s Consumer Protection Act.
17. Employer offers
Employer offers are governed by a separate agreement with the employer. Neither your employer nor its HR team has access to your results, diagnoses or reasons for appointments.
General provisions
18. Governing law
These terms are governed by the laws of Québec and the federal laws of Canada that apply there. Subject to the rights granted to you by law, any dispute falls under the jurisdiction of the courts of Québec.
If a provision is found invalid, the others remain in force. Not exercising a right is not a waiver of that right. If the French and English versions differ, the French version prevails.
